MECHANICS

1. For every 2 packs of Premiere, Frenzy or Trust Condom + One EZ 10g Sachet

2. Ask for a raffle coupon

3. Return it to store clerk after you fill up the coupon and meet and meet all the requirements

Green envelope for Trust Condoms and Trust Fresh Condoms

Yellow envelope for Frenzy Condoms

Red envelope for PREMIERE Condoms

4. The receipt or empty packs/box with the barcode must be inserted inside the raffle envelope as proof of purchase; No receipt/proof of purchase means automatic disqualification of entry.


5. The customer must drop their raffle entries at the drop box located at the participating outlets nationwide.

6. Deadline of entries will be on November 15, 2013, 5 pm.
All dropboxes will be retrieved and sent to DKT Reproductive Health, Inc. office at 80 E. Rodriguez Jr. (C5) Avenue, Bagumbayan, Quezon City.

7. The draw date will be on November 29, 2013 at 3pm at DKT Reproductive Health, Inc. and will be witnessed by a representative from the Food and Drug Administration (FDA).

8. In case of winning more than once, the prize with the higher value shall take precedence. Another winner will be drawn for the prize with the lower value.

9. All grand prizes (mothercycles) registration requirements, insurance and other fees must be shouldered by the winners.

10. All winners will be notified by telephone, text, email and registered mail.

11. Names of winners will be published in major publications with national circulation.

12. Winners will be given ninety (90) days to claim their prizes at DKT Reproductive Health, inc., 80 E. Rodriguez, Jr. Ave. (C5), Bagumbayan, Quezon City. DKT Reproductive Health, Inc. will not shoulder gasoline, transportation expenses, freight and other expenses regarding the transportation of prizes.

13.Winners will provide a copy of at least two (2) valid identification cards and show original copies for verification.

14. A participant must at least be 18 years old to join this promotion.

15.Employees and relatives of DKT Philippines, Inc., DKT Reproductive health, Inc., HealthSense, Inc., IMDS and all of its subsidiaries up to the second degree of affinity and consanguinity are disqualified from participating in this promo.

16. All employees of participating outlets are disqualified for this promo.
